What is the CVC Program?
The Coordinated Veterans Care (CVC) program is for veterans who have served in the Australian Defence Force who hold a Department of Veteran’s Affairs (DVA) Gold Card including war widows, war widowers and dependents.
The program has been established to assist patients, their families and carers, as well as medical professionals, including your CVC Practice Team in offering suitable services to meet your medical needs.
The CVC Practice Team
At Toowoomba Medical Centre our program is facilitated in conjunction with your doctor and our CVC Nurse Co-Ordinator, Rochelle who will work to tailor a program based on your current health requirements. Your personalized health plan will include a current health summary, which will be regularly updated with a medication list, immunisations and other relevant patient history.
Benefits of the CVC Program
As a patient of Toowoomba Medical Centre our medical and reception teams are always willing to assist in your health needs when you require them, however, as a patient of the CVC Program, you will be able to gain one-on-one care from Rochelle (CVC Nurse Co-Ordinator) and your doctor including:
- Assistance in booking appointments, either at the practice or with other health professions
- Regular monitoring of your medical conditions, ensuring that you have appropriate health services made available to you including a GP Management Plan for your chronic health condition and access to Allied Health Professionals as well as a Health Assessment, for those over the age of 75 years
- Home visits from your CVC Nurse Co-ordinator when necessary
- Regular contact from our CVC Nurse Co-ordinator at least once a month, either when you come into the practice, or over the phone, to ensure your health care is suited to your requirements.
Eligibility and Joining the CVC Program
If you are a DVA Gold Card holder and wish to join the CVC Program you will need to speak with your doctor, as well as Rochelle to discuss your eligibility criteria and that you understand the benefits of the program.
