A Home Medication Review (HMR) is a Government subsidised initiative for patients who have been prescribed five (5) or more regular medications.
It is to help you understand the importance of why and when you should take your medication.
At the request of your treating doctor, a pharmacist will contact you to organise a suitable time to come to your home, and review all your medications with you. This appointment will be to assess all prescribed and non-prescribed medications to ensure their efficiency for your conditions.
Once the pharmacist has completed your in-home consultation, a report will be sent to your doctor for review, and you will be recalled for an appointment to discuss the results.
A Home Medication Review is beneficial not only for your health, but for a better understanding of your medications.
