At Toowoomba Medical Centre we offer GP Management Plans for patients who live with a Chronic Disease – an illness or disability that lasts longer that 6 months.
This is a Plan that allows the patient’s medical team (i.e. their doctor, and can include a practice nurse) to assess their current problems and needs, and to set goals to assist in their ongoing health – specific to their needs. Patients are encouraged to review the plan with their doctor every 4 to 6 months.
During your initial visit for a GP Management Plan you will spend 40 minutes with a practice nurse followed by 20 minutes with your doctor. This time allows for our medical staff to assess your current health including diet, exercise, vitals, observations, and medications. It will also allow you time to ask any questions you may have about your condition, or ways to improve your lifestyle.
Other benefits to the plan are:
- A GP Management Plan entitles a patient to five (5) subsidised visits to an allied health professional per calendar year including a Diabetes Educator, Dietician, Podiatrist, Physiotherapist and more. This is an extension of the Plan and is known as a Team Care Arrangement or EPC (Enhanced Primary Care) and you can use these visits in any combination that is relevant to your chronic disease.
- A GP Management Plan, for Medicare card holders is bulk billed, as are the review and nurse visits throughout the year.
