GP Management Plans

A chronic illness is characterized as lasting longer than 6 months for a patient and requires specialized care from more than just their GP. 

A GP Management Plan is initiated by your doctor to allow a multidisciplinary team including the practice nurse, specialists and allied health professionals to all communicate with you and any carers about your current health problems, goals and who else should be involved in your care.

A GP Management Plan also allows for access to a Team Care Arrangement whereby you are eligible to five (5) subsidized visits with Allied Health professionals each calendar year.
