The Department of Veterans’ Affairs (DVA) has created the Coordinated Veterans Care (CVC) program for Gold Card Holders, including veterans, war widows, war widowers and dependents to assist in their management of chronic conditions and complex care needs.
At Toowoomba Medical Centre, our CVC program is facilitated in conjunction with your doctor and Rochelle, our CVC Nurse Co-Ordinator.
The CVC Program revolves around your CVC Practice Team (your doctor and Rochelle) who will work in conjunction with other health providers including specialists, pharmacists, and allied health providers.
To join the CVC program, you will need to speak with your doctor, as well as Rochelle to discuss your eligibility criteria and to make sure that you understand the benefits of the program.
